LASEK Rollercoaster (Journal since March 2007)
I had LASEK Wavefront (not UL) on the 21st of March 2007. I thought I would write down my experience from Week to Week. What I will say is - ignore
the brochures about recovery after LASEK - healing seems to be unporedictable in how it effects different people and for how long. At this point I am
on Week 10 post op and still seeing changes - some good and some bad. Hopfully this might comfort people who are eperiencing the same fluctuations and
who might be getting worried. Having experienced these changes in their graduality I am not going to "rate" how well my vision is until the
6 month mark since I have a feeling their are many more changes to come.
Week 1
Left Eye very blurred, can see severe double image
Right Eye Was very clear but with some "Ghosting" below writing - I had a corneal erosion and vision blurred also.
Close Vision terrible.
Off work all week
Round circle on sky+ box looks like olympic rings
Night vision relatively crisp with left
Week 2
Left Eye still very blurry and double image although the image seems sharp
Right Eye Ghosting still present but distance vision good
Close vision slight improvement but using computer very difficult indeed
Off work all week
Week 3
Left Eye clearer in the morning and then Blurs to previous level
Right Eye good distance - ghosting still present - feels very Sticky
Improvement in close vision but more in left than in right
Off work all week
The 20/20 reading in my left my optom is declaring is highly innacurate since I cannot see writing further than the eye chart
Week 4
Pretty much the same as Week 3.
Close vision improved again
Started work again this week
Week 5
Few improvements - left eye still blurring as day progresses
Close vision still improving but more in left than right.
Right eye ghosting is more pronounced
Week 6
Big improvements - Left eye blur only reaching about half of previous level
Right eye ghosting still present
Startbursts developing in both eyes
Week 7
Left eye continues to improve however:-
Ghosting (or might be called Glare) is developing in both eyes (looks like an aura around objects in poortly lit room)
Startburst developing further
Night vision not so great
Week 8
Left eye seems to have settled, double vision nearly gone in this eye but not 20/20 vision
Right eye seems about the same but with a little more ghosting
Week 9
Steroid drops have stopped at start of week
Very little change in terms of acuity
Week 10
Start of Week Ghosting / Glare more pronounced - seems to be getting worse
Mid week - Acuity in left increased to nearly 20/20 on one morning but then blurred (seems to be an improvement) Ghosting in right declined a little
today but this might be a one off. All drops stopped. I have discovered that ViscoTears actually makes my eye drier in the morning more than if I
didnt take them. Never had a big problem with dryness just feels a little sticky in the right on occasion.
This seems to be similar to my PRK procedure done 7 years ago - ghosting developed at about 2 months. My PRK procedure however, I only had steroid
drops for 2 weeks. I was on steroids for 2 months tapered this time.
This recoery is very different to my PRK - much more gradual (probably due to the steroids)
My close vision at the minute is fairly good, although the actuity difference in both eyes causing slighty annoyance as the image doesnt fuse
correctly yet, but this is improving. I have been patching the left for 15 mins each day to force the right to do more work (I had problems with this
with the right since my PRK).

I was -1 in the left and -.75 in the right. (After 7 years regressed a bit).
You may wonder why I had treatment at this prescription, well, it was to attempt to repair the night vision issues caused by my PRK treatment 7 years
ago as I felt the technology was getting there.
Before my PRK 7 years ago I was -3.5 in both eyes.
My pupils are 8.5mm in low light and my corneal thickness before the op was 430 microns (very, very thin).

Laser Surgery in General
Just a quick word about my emotional state:
Between week 1-5 I severely regretted this "retreatment". I was very down and depressed and had severe regrets as the vision was apauling in
general.
Now that there is a great deal improvement, my emotional state is much better but Im still having some regret as the ghosting disturbances are
"my old friends" coming back in force. I realise however these can just as easily resolve and they have improved a little today in fact.
You have to be very strong mentally for this procedure (I have done this twice now) and it takes a lot out of you - for months, and you are constantly
reminded of your situation. *Try to forget about it* is just a platitude that my optoms in the past have used. I very much doubt that anyone
experiencing the early postoperative disturbances could simply *forget about it*.
Some optomoetrists (no reference to UL BTW here - I have not had experience of them) I feel dont appreciate the emotional journey and I feel more
counceling services are needed throughout this, not just an optom telling you your fine when you know that in general you are certainly not fine.
After care should involve a counceling help line or other counceling services, to at least make you aware that there is an informed person to talk to
if you need them who are not necessaraly totally academic and full of platitudes about the whole situation.
Laser treatment services and postoperative care still has lessons to learn in terms of making the patient feel comfortable.
I am however, very pleased for the people that get good results early.
Although I didnt go with ultralase, I think that the fact that they have provided this forum is an excellent postoperative support tool. We all have
opinions about laser eye surgery.
My opinions are perhaps at conflict and are as follows:
* The laser technology is fabulous and highly advanced
* The surgeons (I had) are very highly skilled
* The human healing process is unpredictable between patients, eyes, current curcumstances. So many variable - so unpredictable.
* With the greatest of respect to surgeons and optomotrists who are infinately more intelligent than me, I am acutely aware that there is a
significant factor in the healing process that is simply not understood and as such laser providers are unable to give patients the degree of
re-assurance they need. This might be an unpopular appraisal of the situation but I firmly believe this to be true.
* I had no option but to have LASEK as my corneas were too thin for LASIK. However, I wound not have had LASIK anyway as I rub my eyes quite a bit and
uncoinsously sometimes - my flaps would have been gone in a week im sure.
* There is currently no perfect procedure. LASIK is great for healing, LASEK is good for long term corneal integrity. LASEK has been touted as best of
both wolrds but it isnt really because the healing is still lengthy and upsetting and in my opinion is the same, if not worse than PRK.
* The fact that after six months you might have good vision is of very little comfort - this is a long time out of anyones life.
* Having had PRK in the past I can tell you that in my experience LASEK has absolutetly no benefit over PRK. I beliebe that the LASEK epethilial flap,
which dies and regenerates, actually causes signifcantly distored vision for several weeks since your walking around with partially dead corneas for
several weeks that are likely to be very, very rough.
* It doesnt matter what you have done - your healing will dictate the result.
* If your laser does not have an eye tracker, forget it - your corneas will probably look like the himalayas. Humans cannot "fixate" easily
until they are dead. In my pRK - the laser did not have a tracker and my right eye moved upwards. THe topography I saw 7 years later very expectadly
showed what looked like the alps in the lower region of the cornea. I have always had *unnatural* vision in this eye since the original PRK
procedure.
* LASEK / PRK is on the whole probably less risky than LASIK. Flap complications can be severe as well as dry eye, so I have read through my own
research. I also know someone with LASIK flap complications that are not correctable. Microstrae in his flap causes him ghosting and glare.He had a
flap lift but it made it worse.
* If you need to work with detail (computer users / architects) consider the effects on your career of a poor outcome.
* There is a lot more to seeing that the mechanics of the eye and cornea. Much is done in your head and a significant change to your visual inputs
will mean you need to relearn how to see and more importantly comprehend (probably especially if you are a higher myope / hyperope). When I had my PRK
in 2000, I could not comprehend images correctly for a very long time, although the images were clear.
My brain was not able to correctly interpret what I saw after PRK, there seemed to be an overload of information that I could not percieve. This took
years and years to improve. My LASEK has very slightly improved how well I can interpret what I see. I used to be able to look at a whole screen of
text and pick a single word out in a couple of seconds. Since laser sruegry I need to read each word on each line individualy. Im not sure if the
image has changed, but my head cannot process easily a screen of text and all of the extra peripheral *data*. This effect is something that my optoms
(Not UL) did not acknowledge. I guess this may come from the way the brain settles on its own procedure for looking, seeing and comprehending. I
believe Laser surgery may change this significantly to the extent that the brain needs to relearn how to comprehend visual inputs properly. They say
if you wear a pair of glasses that flips the image upside down your brain will eventually point the image the right way up. People with already poor
visual comprehension in my opinion could be made worse by a laser procedure.
Anyway... As I stated I have had no experience of UltraLase, apart from being turned down on the basis I had a primary PRK procedure elsewhere.
These comments are purely my own opinion about laser surgery in general ,having gone through PRK and LASEK and experiencing the healing of both.

Week 11 - Start
Today is the start of Week 11 post op.
Just in the past few days I have noticed big improvements.
I had a consultation yesterday and the optom said my corneas were completely clear although there still appears to be a lot of healing going on. He
also said the steroid drops might still be in my system.
The good news is I could truly see the eye chart yesterday and it was a vast improvement from my consult a month ago.
On the first chart he said just read as far as you can get to. I said, no need, I can see all of it perfectly clearly. I couldnt even read the top
line of this properly 5 weeks ago due to the double vision as well as other things.
On the second chart.
I could read all of it except the smallest line which is pretty darn good - better than the result in terms of acuity of my PRK. I wonder if I might
be able to read that line at my next consult in another month.
I went out last night to a Pub that I went to a couple of weeks ago. The ghosting was not nearly as bad as it was then although it seems to be
"changing", i.e., sprouting off flares in different directions as opposed to getting worse but Im certainly no longer "glared out"
by all the visual phenomenon.
So perhaps things are starting to look up in terms of low light / night vision phenomenon.
If it stays like this but doesnt worsen I will be fairly happy.

Week 11 - Midweek
I have seen a dramatic improvement in the quality of my daytime vision this weekend. I can see clearer, easier and sharper than I have ever
experienced in my life. Even better than the original PRKs daytime vision result. Distance is just beautiful to look at with great clarity. The
ghosting is still present, I think its got a little better but its difficult to guage that at the minute. If this sudden improvement is anything to go
by I have high hopes for the ghosting to dassapear. I am still marvelling this morning at my vision.
10 and a Half weeks on I didnt expect such a rapid improvement.
4 or 5 weeks ago I thought my left eye would never get clearer and that it would always remain a double vision horrendous blur. Now it seems to be
better than my right eye and beautifully clear.
I must say that if this clarity of day vision remains or improves further I could handle a little ghosting and starbursting even if it is
permanent.
I go on holiday soon. This improvement is perfectly timed - Im really going to enjoy my hols.

Thanks for posting your journey elmoodster. In a strange way its quite reassuring to know somebody else is going through all of this. I had read your
post before the recent dramatic improvements so it was amazing to read those. It seems so strange that after weeks of struggling suddenly within the
space of a few days things improve noticeably. I had a similar experience after about 4-5 weeks in the space of 3 days I noticed my vision was
suddenly much sharper on waking. It deteriorates a bit through the day and still isn’t 20/20 but from where it was the change was amazing. But it
seems to have stopped there for the moment and again I’m wondering if this is it. I assume not and I have heard other people say sometimes improvement
can occur in steps rather than slowly and continuously! This is indeed a very strange rollercoaster we are on!

Week 15
I started Week 15 post op on Wednesday. Apologies for the gap. At week 12 I went on Holiday for 2 weeks.
During my holiday nothing eventfull happened with my vision apart from my eyes went quite dry when I arrived but that only lasted a short period of
time. My eyesight wasnt spectacular during this period but the sun was quite strong and I wore my glasses all the time, but wasnt a problem really. No
problem in the swimming pool / at the beach.
One strange thing, when I got salt water into my eyes the sight seemed to improve for a short period of time - strange.
Now I would say it was a few days ago I noticed another improvement in acuity in my left eye so it may end up exceeding 20/20.
Still have a few issues with right eye and reading.
My biggest problem at the minute is ghosting / flare. This seems to be continuing to get worse but I have read many articles to suggest that this
reaches a climax at between 3 - 4 months and then improves. I hope it does - its quite disturbing. I did have ghjostin after my inital PRK 7 years ago
which I have to say was a lot worse than this current experience. I think with the PRK it did seem to get worse then improve a bit but I was left with
persistent ghosting effecr hence my choice to have wavefront guided LASEK.
I believe that my optical zone may be shrinking due to healing. My large pupils will almost certainly be making this effect worse. Hopefully as the
cornea smooths out the effect will become less although I am resigned to the fact I may be left with some ghosting as with my initial procedure 7
years ago.
Bear in mind my ghosting really started to develop between 2 and 3 months so this might be something to bear in mind when you have surgery - i.e.
expect some changes that may disturb you down the line.
I can say though that I am generally happy in myself. This is the first week I have found it hard to recall how many weeks ago it was - perhaps I am
not thinking about it as much.
One thing is clear (forgive the pun). It doesnt seem to matter if you have wavefront or standard treatment. Your own healing could entirely negate the
benefits of wavefront in visual quality terms, which seems to be happening to me now with the development of this ghosting abberation.
I did have to have wavefront anyway as in my case it was going to spare some more tissue and was the best option to avoid perhaps more serious visual
defects.
Well, I have an aftercare on July 7. Im going to rant on about my ghosting again and no doubt annoy my optom who wont be able to do a thing about
it.
I feel a bit like a sausage with my laser provider (not UL). Some of the optoms have appaulng skills when it comes to putting minds at rest. They seem
to mention the word retreatment a bit too often for my liking.

Week 16 - Bad News
Well, Its Week 16 now and im afraid things are really starting to go downhill. Im not talking about my acuity here but my ghosting, starburst, flare,
haloes (Oh yes I have all of them now). These have got signicantly worse this week. They are at the level I had before this second surgery making the
entire excercise absolutely pointless. Reading is quite difficult still in my right eye.

Where to from here
I have an appointment with my Optom on Saturday. I will either be asking for any new medicines to control pupil size or for abberation control contact
lenses (that may need to be over minused - I have no prescription). I am bitterly dissapointed right now and I feel that nobody is really trying to
help me. Its quite possible my vision will be in this butchered state for the rest of my life. Why did I do this.

Week 16 Update
I had an after care yesterday. I drew lots of images of what I was seeing. The optom didnt seemed phased by this as if it was "normal".
Seeing an LED that looks like a Leaf is not normal to me.
Anyway I asked a barrage of questions about my pupil size, transition zone etc.
He absolutely insisted that there were no irregularities in my Cornea and that they were quite spherical. There is no prescription in either eye, not
even slight astigmatism.
He stained my eyes and suggested that my Tear Film is of a high quality but very poor quantity, i.e. Dry Eye. He has told me to stop using ViscoTears
altogether as this could be retarding my Tear Film production.
I still have difficulty accepting that this is causing my Ghosting. It may be making it worse but in my opinion it is not the cause.
I suggested a Wavefront scan but he said that there was no need for this.
Im going back in a month and suggested that they may consider temporary punctal plugs. The wierd thing is that my eyes dont burn, they arent red, they
may be a "*little*" sticky and uncomfortable.
This has infuriated me because the optom I saw prior to this was talking about enhancment surgery.
Im confused and distrustfull at the moment.
I personnaly believe that Light is scattering through the transition zone and creating the ghosts, getting worse over time because my optical zone is
shrinking during healing.

Week 17 - Very, Very Upset
Things are getting worse and worse. My left eye is like vaseline has been smudged over it.
I couldnt wait any longer so I went for the punctal plugs fitting yesterday.
Ill tell you more but first just let me say that I rung the Clinic and the Manager spoke to me as I asked to see the surgeon (PLEASE BE AWARE MY
PROVIDER IS NOT ULTRALASE). She was intensly rude to me and and told me that she had 15 patients waiting for surgery, they were very busy and the
surgeon cant see me (she didnt offer another day). I explained my situation and she simply said "He wont be able to do anything about that
anyway". I was disgusted at her approach with me, which I proceeded to tell her. Anyway I managed to get her to agree for me to come in to have
punctal plugs inserted (still not convinced of Dry eye I might add).
I got a different optom when I went. Thank god I did because I have finally found out the truth and essentially discovered I have been lied to up
until this point.
This time the optom told me my tear film was stable and I dont need plugs but I must continue with the ViscoTears (the other one told me to stop!).
Whats more distressting is that he discovered that I have blepharitis and gave me all the instructions on how to clean my eyes every day with baby
shampoo etc. I have a lot of soreness around my right eye and in my cheek. I can feel my eye moving. I have felt like that for years. It is possible
that I had blepharitis when I went under the laser. Why this wasnt detected I have no idea. The other otpom told me my eyes were fine.
The optom did lots and lots of tests including a pin hole test. I could see clearly thorugh the pin holes. He also did a Wavefront Scan and heres
where we get to the heart of the matter with my ghosting.
He told me I had 97.5% HOA in my right eye. (This was 57% prior to surgery). Essentially the surgery has made worse what I went in to get treated.
My vision in dim light is now shockingly horrendous like horrid vaseline smudgy horrible mess. I feel intensly depressed and totally angry with my
laser clinic.
The optom tried some contact lenses but non of them go rid of the ghosting. Although he has booked me in for a fitting in a couple of weeks time to
try the Wavefront / Aspheric contacts. At least this guy was much more helpful, trying to get to the heart of my problems.
He didnt want to try Alphagen yet (which I accepted as this guy was really helpful) and he said that I need to get the blepharitis under control
first.
The optom did suggest a potential wavefront re-treatment to remove the HOAs but these things can develop after surgery so to me there is absokutely no
point and great risk of things getting worse (remember I want in to get my HOAs treated in the first place due to a previous PRK going wrong in this
way).
Im still considered a success, by the way. The optom did say that these HOAs could get better with time but from my perspective they are simply
getting worse. My hopes for a happy future were pinned on this surgery.
Im going to the doctors today to get myself checked out. I have been befroe about th epain in my face - they just said it was eye strain - now im
going to ask them to test me fully. I have always been able to feel my right eye moving in my head as if 2 non lubricated surfaces are rubbing
together.

Ultralase
Can I just say that I rung ultralase for advice about my situation (it was Toni from the forum) and she was very, very helpful in terms of giving me
some advice. I thought this was very kind of her since ultralase are not my provider.
I really wish I had gone to ultralase in the first place 7 years ago.
If your reading this thanks Toni for your advice on the Phone yesterday.

In terms of how people feel when they are reading my journal, please bear in mind I was never really a candidate for this surgery 7 years ago.
(Knowing what I know now) I only wish my clinic had refused me in the first place.
If your considering Ultralase then I am absolutely certain that you will be selected and properly.

Week 17 - Mid Week
Hi,
I just spent an hour writing something to go here and then deleted it because it was far too strong, especially for a site that is providing me with a
forum for conversation.
Take my advice GO WITH ULTRALASE if your considering surgery, i feel at least after speaking with toniK that they have a level of integrity and
professionalism that surpass the other clinics. In the 5 mins I spoke with Toni, hardly anyone has given me any such sincere consideration at my
clinic.
ALSO - If ultralase turn you down for surgery, count this as a blessing that may have saved you from a serious mistake.
I wish I had gone to ultralase and that they turned me down, as the other clinic should have done.

Interesting
Hi, Ive found out something that might interest others. I took Nurofen Plus for some pain in my right eye socket. This eased the pain. I went out
tonight for a curry. I noticed a reduction in ghosting effect whilst I was out.
Ive just done some research and found that Codeine (present in nurofen) can induce Miosis (constriction of the pupil).
This is significant, I had a meal and enjoyed myself. I could still see the ghosting but it didnt seem to interfere with my enjoyment. The ghosting
was reduced to a slight glow around objects which was manageable.
I havent seen this mentioned anywhere on the internet in terms of a method of approaching dim light problems.
Perhaps I have hit on something here. If your having ghosting and starburst. Try a nurofen and see if it gets any better. This would be an interesting
find if people benefit from this. I will have a few tablets tomorrow night and report back.

Update
Hi - Just an update during my lunch.
The nerofen was a red herring. The same effect didnt happen again. Its probably that I was just happier with lest pain around the globe of my eye. The
ghosting is actually getting worse over time.
Anyway. Possibly I was a little down over the weekend. Its easy to get into a cycle of getting yourself down even further when thats all you end up
thinking about.
I have an appointment on the 24th. Hopefully my inflammation should have cleared by then. That will be the 4 month mark and I so hope that thinks
start to improve or that I can be offered alternatives to correct my HOAs. I will NOT undergo another surgery as I am certain this it WILL cause the
problem to get worse and not better.
I have hopes for a future contact lense fitting, which might happen on the 24th provding my inflammation has settled by then.

My refraction is still pano in my right by the way with so very slight virtually undetectable astigmatism in my left. Several scans were done to try
detect it.
I can detect that astigmatism at a distance but its difficult to tell when its mixed in the ghosting if its present close up.

Sorry to hear of your experiences elmoodster, but am glad you're making the effort to post here. You make some good points and it would be
interesting to know which provider you went with, or at least which laser was used for your LASEK.
I've mentioned here before about the importance of the laser used when you have larger than usual pupils, but sadly some people don't seem
to realise this, thinking all lasers are the same. Of course understanding the technical details behind this is perhaps beyond some people, and
something they may only appreciate with hindsight.
I wasn't aware of the potential miotic effect of codeine, I see that it may also decrease intraocular pressure, increase accommodation and
increase light sensitivity.

Hi Gezi,
The strangest thing is that the wavefront ablation did get rid of the ghosting initially, (double vision was worst in the early stages due to
astigmatism). The ghosting is now "growing back". Mostly likely because cells are remodelling in the periphery and negating the effect of
the ablation.
I think pupil size is absolutely vital as you say. My initial clinic that did my PRK should have refused me in the first place but at that time I did
not understand or read anywhere about Pipil size even being an issue in laser surgery. I, like so many others, trusted medical professionals like a
baby trusts its mother. That is not the case now, quite the opposite.

Well hopefully the Alphagan drops will provide you with some temporary relief from night vision problems, although apparently its miotic effect does
diminish if used for a while. I've used it on an ad hoc basis and I do wish my night vision was always like that, but of course I accept the
compromise.

As Glenn points out "better is the enemy of good enough", in the area of vision problems, striving for perfection can just make things
worse...

VISX Star 4 IR (LASEK)

A different outlook
I have obviously been talking alot about my problems here lately.
I have come to realise that perhaps I am wallowing in too much self pitty about this.
My decision to undergo laser surgery was indeed my decision.
Nothing would have stopped me having LASEK, im just that kind of individual that must attempt to put something right when its gone wrong. I should
have realised from my first encounter with PRK that its not that simple with Refractive Surgery. Its all about healing also. Perhaps my problems are
actually the way in which my body responds to an insult or injury.
I read something just a few minutes ago that reminded me that although I have horrible distrubances that I am still able to do things in my life and
thus I am not "disabled", I just need to finally accept that I will need to adjust my life to this new vision. I should have accepted that
after my PRK really, but I didnt and that is nobody's fault but mine and I will have to get used to it.
Some people choose to shoot down the LASIK / LRS industry as a result of a bad outcome. I am not going to do that but at least if people look at this
post on here it will allow them perhaps to make a better judgement than I did.
There are so many people who have real disability in life through absolutely no choice of their own.
I had a choice and even though I have bad outcomes I am not disabled and not nearly as unfortunate as so other many poor people.
So tomorrow is a new day, with a new way.
I will still pursue the Contact Lens option but whatever comes of it I need to get back to my responsibilities, this isnt just my life I have
suspended by my problems, its my partners also. I still have the options of Alphagan and Contact Lens fitting so ill just keep my fingers crossed, but
not too tightly. There is of course still a change that things might improve for me.
I still post on here weekly giving you an update, but without the self pitty that has gone with it over these few weeks.

Question
I have a pear of specs of prescription -1D. If I wear them my ghosting greatly improves. My close vision goes a bit haywire. Would it hurt my eyes
which have no prescription, to wear these glasses on a permanent basis?
